Office Policies

In order to provide the highest quality care we have listened to your requests for less waiting time.  To develop this we have adopted the following formal policies: Late, Missed, Canceled and Phone Call.

Affleck Eye Care values your patronage and is committed to exceptional medical care and service. We respect the importance of your time and work very hard to schedule appointments that accommodate the busy scheduling needs of all of our patients. Because we do not schedule several patients at the same time, all appointments are reserved exclusively for you. In return, we ask that you make every effort not to missed appointments or be late. Late arrivals, missed appointments and other delays place a burden on the entire staff and those waiting. The appointment timetable is skewed creating longer wait times for other patients as well as the practice.

Late for Appointment Policy

We certainly understand that unavoidable delays occur in arriving on time for your appointment. Unfortunately, we also become delayed due to unforeseen problems with patients or when a family requires extra time for an appointment. This places us in a position of not being able to see you at an appointed time. However, we make every attempt to ensure patients are seen on time and in the order of their arrival. Affleck Eye Care requests that you be on time for your appointment.

If you are a new patient, we request that you arrive 10 minutes (45 minutes if you have NOT fill out your patient information form) before your appointment so that our staff can properly register you.

If you anticipate being late for your appointment, please call us and let us know. If you fail to arrive on time for your appointment and are less than 15 minutes late, we will try to see you; however your visit may not be completed and another appointment may need to be scheduled.

Please note you will only be able to be treated for the remaining time originally allotted for your scheduled appointment time.

If you are more than 15 minutes late for your appointment, you will need to be rescheduled for a later date. However, if there are extenuating circumstances you may possibly be seen.

Missed and Cancelled Appointment Policy

In order to be respectful of the medical needs of other patients, please be courteous and call Dr. Affleck’s office promptly if you are unable to show up for an appointment. Appointments are in high demand, and your early cancellation will give our office time to reallocate your exam time slot and give another person the possibility to have access to timely medical care.

We require at least 24 hours advance notice that you will be cancelling an appointment. A twenty five dollar charge may be applied for missed or cancelled appointments without advanced notification. Payment for a missed appointment is the responsibility of the patient and cannot be billed to Medicare or other insurer, and is due upon receipt of the bill.

Chronic missed appointment is a critical concern and may interfere with your health care needs. It may also result in patient dismissal. 
Phone Call policy
Patients are encouraged to call the office for any problems; however, medical questions are best addressed by consultation with Dr. Affleck at the time of your appointment. For Dr. Affleck to leave a patient being treated to answer phone calls would obviously be unfair to the patient being treated. The office staff has been trained to answer most questions. If they are unable to help you immediately, the staff will return your call within 24 business hours, unless unseen circumstances. Usually specific questions will require charts to be reviewed and discussed with Dr. Affleck prior to the call being returned. This is efficient and results in better patient care. Should you have an emergency, please notify the staff.

Insufficient Funds

Payments made by check to Affleck, MD Eye Care that are not honored by the bank will incur a returned check fee of $10 or more.  This amount is subject to change.  Affleck, MD Eye Care will not accept checks as payment if two checks have been returned for insufficient funds.

Glass Prescriptions are NOT Guaranteed

We apologize for any inconvenience, but since Affleck MD, Eye Care does not have an optical shop, the person who dispenses you eyeglasses or contacts is responsible for their accuracy.
